How do I schedule my first appointment? Most Wellness Rx providers can see a new patient within the week you call. However, Dr. Jay Nielsen is scheduling 5 or 6 weeks in advance. Dr. Nielsen requires a $100.00 deposit before your 1 ½ hour new patient appointment can be scheduled. We will hold your cash or check deposit until your first appointment and return it to you that day. You can also provide a credit card number that will be held until the day of your first appointment then destroyed. Your deposit will be returned if we receive at least 24 business hours notice of your cancellation. Simply call our office at 419-897-6490 for scheduling information. What are Dr. Nielsen's credentials? Dr. Nielsen is a Board Certified Family Physician, trained at Toledo Hospital, who has done non-surgical orthopedics since 1981 and has specialized in nutritional medicine, hormones, and detoxification for pain relief since 1993. He uses supplements, exercise, nutritional injections, manipulation, physical therapy and comprehensive history, exam and diagnostics to find the cause of pain. How do I obtain prescription refills? Prescriptions for controlled drugs require that you be seen in our office on a regular basis. You will be instructed at the time your prescription is first written to schedule follow up appointments. Refills for compounded prescriptions may require routine lab testing and your pharmacy will need several days to prepare your prescription. Contact our office 14 days before your prescription is due for refill. Routine refills may only require a phone call to our office. Please provide 5 business days notice. Are Wellnes Rx providers in my insurance network? Not all Wellness Rx providers participate with the same networks. We recommend that you contact your insurance carrier before scheduling an appointment as out-of-network penalties could apply. Is payment required at the time of service? Co-payments and deductibles for covered services are due at the time of service. Payment is required in full at the time of service for non-covered procedures such as Prolotherapy, Vitamin C IV’s, Meyer’s Cocktails, Nutritional Counseling, etc. The front office is staffed from 8:15am to 5:00pm Monday thru Friday. The front office telephone hours are 8:15am to 5:00pm Monday, Tuesday, Thursday, and Friday. If you need to contact the office on Wednesday, simply leave a message with the answering service and your call will be returned within 24 hours. Ohio Bureau of Workers Compensation Information As certified Ohio BWC providers, we can manage the care of your industrial injury. You must file form C23, Change of Physician, before your first appointment. This will inform the Bureau of your intent to change physicians and assure the payment of your office visits. Your plan of care may include diagnostic testing, surgical consultation, physical therapy, chiropractic treatment or other care. With few exceptions, pre-authorization is required for all testing and treatment. Our office will file the appropriate requests and schedule your appointments when approved. Obtaining treatment without prior authorization may result in non-payment of your claims. If your injury prevents you from working, our office will file form C84, Request for Temporary Total Compensation. Please provide one week’s notice if your compensation needs to be extended beyond the expiration date of the previous request.
